Healthy Teton County is an initiative that began in 2013 with a series of conversations between St. John's Health (SJH) and the Teton County Health Department (TCHD). Federal Affordable Care Act legislation mandates all hospitals wishing to maintain 501(c)(3) status participate in a community health needs assessment and create an implementation plan based on the results. St. John's Health is subject to these requirements. At the same time, TCHD was mobilizing to collect data and update their biannual health assessment of the county's population.
The two agencies decided to combine efforts into the first large comprehensive health needs assessment for the community. Community Health Needs Assessments have been published in 2015, 2018, and 2021. For the 2024 Community Health Needs Assessment HTC will be using the updated MAPP framework, or MAPP 2.0. The updates to the MAPP framework focus on health equity.
In May 2015, the Healthy Teton County initiative published its first comprehensive community health needs assessment (CHNA) for Teton County, Wyoming. This project—led by the Teton County Health Department and St. John’s Health, in partnership with over 35 community organizations—identified and prioritized the primary health issues facing Teton County.
